How do I apply for Mobile Meals?
You, a family member or a caregiver may apply for services. You may apply on-line or download a form to fax or mail. You may also telephone (800) 852-6325, (330) 376-7717 or (440) 234-2433. A case manager will speak with you to ask your name, address, health status, etc., explain our services, determine your eligibility and confirm the type of service needed and the appropriate meal plan.
